Abstract | There is a rationale for primary prevention in relation to health promotion in children if the following hypotheses are valid: 1) A sedentary lifestyle causes increased levels in disease risk factors, which are known to increase risk of premature death. 2) A large percentage of European children have a sedentary lifestyle to a degree that it may increase the risk of developing atherosclerosis and lifestyle diseases such as CVD and type II diabetes prematurely. 3) Risk factors or sedentary behaviour track during childhood and into adulthood. 4) Interventions including increased physical activity in children at risk are efficient in decreasing risk factor levels and changing behaviour to a more physically active lifestyle. In this chapter, the authors argue that the above hypotheses are true. The chapter focuses on: i) physical activity in children and youth: recommendations and determinants; ii) key mechanisms, which link physical activity to the biological CVD risk factors including the atherosclerotic process; iii) associations between physical activity/fitness and CVD risk factors; iv) tracking of physical activity, fitness and CVD risk factors; v) what can be done; and vi) concluding remarks.
